Broken condom? Diaphragm fell out and nobody noticed? Premature ejaculation and the guy doesn't admit that he dumped his entire load inside the character? Or just the good old "It felt too good to pull out, oops."

Accidents happen. Sometimes the character may notice the sticky mess down there, sometimes the character may go on with her daily routine without even noticing the droplet of sperm that had gotten into her. And that can be all it takes to turn her into a mommy. Ignorance is bliss, or is it?
