The game keeps on going until the baby is born though many are more preoccupied once the water breaks, once the baby is out the new reality sets in with them having been remembered as the mother permanently. With the pregnancy there are often unfortunate side effects; clothes often tear for new mothers not expecting a sudden baby bump, the rush of hormones can cause increase in libido, and the sudden shift can make accidental lactation very common. Most common is people playing will see themselves as still pregnant even if they tagged someone else

There are some important rules to considered often left off the pregnancy tag. Players aren't allowed to enlist help from others to tag another target. They're not allowed to pay or someone to get tagged whoever is it has to be tagged belly to belly no . Players have to bear being a mother for at least 24 hours until they're allowed to tag another target. Finally, more of a warning the game of tag can have an odd effect on already pregnant women who are tagged the game doesn't seem to recognize that a mother might want to hold onto the child she started with and upon tagging another she might find herself without any child and the game of tag now has multiple children in play with it being difficult to separate them.

Players can make their own rules but they must be added to the tag before playing, this is referred to as playing with a twist. The rules can change how the game is played to an extent , for example; you can extend or shorten the timer between tags, make it so an objective has to be completed (Streak, pump milk, get laid, have a number of people rub your belly, etc),make it so certain behavior has to be done while tagged or make certain actions count as cheating or add unique punishments. However no new rules added are allowed to prevent play, i.e you could extend the timer but not so far that the person tagged can't tag another or perform an objective that can't be done.

If someone breaks the rules the punishment can be severe ranging from sudden surge in libido, growth in breasts and belly, extending the timer before they can tag, pushing the pregnancy up a month or increasing the amount of kids. If players tag each other back and forth repeatedly the game may punish them by making both players pregnant which can cause the game to spiral out of control.
