Misato knew Shinji would keep coming up with excuses to avoid getting nude, so she did what she thought she had to do.

She took Shinji's underwear with her right hand and pulled it back but didn't took her own strength in consideration. Shinji noticed this and moved in the opposite direction. Meanwhile, Ritsuko was surprised by Misato's boldness.

What happened was that all this movement ended up in Misato accidentally ripped off Shinji's underwear, exposing Shinji in front of all the girls present.

Everyone gasped at this, including Shinji, who felt paralized by shame and was unable to move. Despite technically succeding at doing what she wanted, Misato felt bad for ruining the Third Child's underwear.
