Dragons being demi-mortal apex predators composed mostly of magic, have a profound effect on any environment in which they choose to make themselves at home. Their excrement is the primary source of all mana, their urine is the catalyst for monster genesis, their borrows are tunneled through the fabric of the material plane deep into extra dimensional space over which the dragons will build a dungeon they will populate with highly dangerous and poorly contained monsters.

Climate, seasons, and weather are controlled by the mechinations of the gods and the behavior of wild elementals. There are lands known for their endless summers, eternal winters, perpetual springs, and unending autumns, but just as many places have the cycle of seasons in length comparable to the years on the long forgotten planets for which the human populations originate.

Landmasses rise and fall in ever changing mosaic. Though to the mortal reckoning of time the mountains and valleys seem eternal, to the reckoning of the gods and dragons, whole planet sized continents come and go like carrots bobbing on the surface of a soup.

Arch mages and high clerics are always creating new islands or summoning continents from distant planets, along with their human populations. There are always new lands to explore, new frontiers to settle, new monsters to domesticate.
