At this point, you realize you're no longer wearing underwear. A quick analysis of the room spots them, in a corner next to a trash can. A trash can filled up with used gauze, that has no blood on it, and wasn't there before. The distinctive smell in the air is the piece falling into place that makes you understand what happened here.
