You dig through your pockets for the slip of paper that you had earlier and she takes it from your hand before tucking the small sheet of paper in her hair between the black cat ears that were now on her head. You couldn't see any human ears on her so you couldn't tell if she had two sets of ears.

"Now, I am going to need some personal information about you. You can make a couple of changes to yourself here if you want, I don't mind. Anything you write here though becomes true and if you make it to the end of the road, you can make these changes permanent in your world as well."

The cat girl then picks up a clipboard with a some papers on it and a pen attached to a string with her tail. Her black tail daftly tosses the clipboard over to you, which you fortunately catch. Looking down you see some spaces to fill out some information about yourself.
