Destiny time? Or more interruptions?

Welcome to Apertur-- I mean... Illumination Laboratories...

The train pulls to a stop at the right station, and you hop off again, walking briskly and with purpose towards your soon-to-be new job. You smile confidently to yourself, sticking out your chest slightly as you walk through the front doors of Illumination Laboratories, heading straight for the front desk.

"Excuse me."

The clerk looks up at you. She looks you over, raising an eyebrow. "May I help you?"

"I received a flyer about volunteering for a test trial of some sort?"

Realization lights up in her eyes. "Oh! You're one of the ones Dr. Weishaupt selected!" You tilt your head slightly. The flyer hadn't mentioned the name of the doctor, so you weren't certain if that statement was accurate. "He will be delighted that you've arrived. Please, take a seat. I'll page him for you. What is your name, dear?"

"John Doe," you respond, glad to see that you are almost certainly going to get paid in no time. You do as instructed as she does as promised. It only takes a couple of minutes for a woman only a few years older than you to open a door, calling out for you. Hurriedly, you stand and walk over to meet her.

"Hello, Miss Doe," the woman begins, shaking your hand. "I am Minerva, Dr. Weishaupt's assistant."

"Hello." You smile broadly.

"Please, follow me." The nurse leads you through a series of winding corridors, eventually stopping outside of an unassuming room. "The doctor will meet with you shortly. As part of our trial, you will need to complete a short survey first. It will be pulled up on the computer inside this room. We will need you to be perfectly honest with us. Don't try to tell us what you think we want to hear."

"Okay," you agree, nodding your head. She opens the door for you, and you enter. For a moment, nervousness strikes you. The room is illuminated only by the computer in question, and you think you hear the nurse locking the door behind you. Swallowing the lump trying to form in your throat, you step forward confidently, taking a seat before the digital questionnaire. There seemed to be no mouse, so you worked with the assumption that it was a touchscreen.

Despite yourself, you raise an eyebrow at the first question you see:
