When she hugs him

"Oh, does she hug you often? When you come home, maybe?"

"Not that often, I guess? Short hugs like that aren't as good anyway, I guess it's more like when we, uh..."

"When you cuddle?" she supplied, and he nodded, a little embarrassed by the phrase. "That's very good, it's a very healthy thing to do in a relationship, just like being in love." She watched as a tinge of color appeared on his cheeks from her implication that he was in love. "Now, I'm going to give you an instruction that you should try out before our next appointment."

"Uh, our next appointment?" he asked, not having expected that.

"Of course! I can't fix your emotional distance in one day. In fact, most studies show that even people who don't have a specific problem benefit from seeing a therapist regularly, it's like going to a gym." She chose a metaphor that might appeal to someone as concerned with appearances as he was.

"Okay, so what do you want me to do?"
