Which door do you enter?

Gentlemen

You enter the door marked for the use of men, and immediately encounter a wave of pleasantly scented warm air. You leave your coat on the rack provided and proceed through the only open doorway. A young woman seated behind a mahogany desk smiles at you.

"Mr. Doe, I presume?" At your nod she gathers a sheaf of papers and a pen for you. "You need to fill out all of these forms before your appointment, sir. May I offer you tea? Coffee?" She sets the forms and your drink on a side table and motions for you to sit in an overstuffed chair, into which you sink gratefully. The manor is filled with nothing but the most expensive and pleasing furniture, and as you sip your drink and take up your pen you decide you have made the right decision.

The form requests the most basic possible information at first -- name, age, vital statistics, address -- but as you proceed through it the questions become stranger. This is the first one to which you must give considerable thought:

Would you prefer to spend your time here in a different physical form?
