When she is just a few inches away from you, she stops. You look her quickly up-&-down, before speaking.

"I think it's a bit ironic to be getting a physical on my birthday!" you remark, laughing.

"Relax, John !" she tells you. "I know it's your birthday, & I'm not here to give you a physical!"

"Well then, why ARE you here?!" you ask her.

"I'll tell you when we get in the room!" she shouts back.

"Great idea," you tell her. "But the door didn't open," you add, holding-up the card.

"That's not the key!" she says.

She then points the gun at the doorknob, & pulls the trigger. Out pops what appears to be a bullet, before it unfolds into what appears to be some sort of mini-credit card. She swipes it, & the door opens.

"After you, Birthday Boy!" she says, & you head in, guessing that she remains close behind you.

Soon, you look ahead of you, noticing what appears to actually be a hospital bed with a bedside table & some sort of currently-open curtain nearby. You turn to face her again, just in time to watch the door close behind the two of you.

"I...," you start. "I thought you said you weren't here to give me a physical!"
