What state has she been kept in?

Isolated and haggard

As you go down flight after flight of stairs the air turns cold and damp, the corridors only lit up by small lanterns and the prisoners kept in the cells left in the dark for most of their days here.
Once you reach the last level you walk down the corridor and are met by a older guard. He´s sitting near the stairs in a plain wodden chair with a single torch over him, seemingly busying himself with carving a small wodden figurine. When you enter he quicklt stands up and salutes. "Greetings General!" You ask him which cell the traitorus cavalry officer is held in. He grabs the torch and beckons you to follow him. Down the corridor, third on the left, he stops at a metal door. Its not all solid metal, but made like a large metal grate. Looking through, you see what´s become of your officer.

She´s chained to the back wall by her wrists, ankles and neck. Even though its pitch black without the torch, she´s also got a crude leather hood laced over her head, muffling her and blinding her. She has no clothes, lettig you see how much she´s shrunk. Before she was a muscular woman, able to outperform most of the male soldiers under your command, now you doubted if she could even take down one of your servants.

The guard lights a lantern hanging on the wall and goes back to his chair, resuming his wood carving. You crouch down and hiss at her. "Get up!" She stirs, groans and gets up into a kneeling position, but does not stand. "I said. Get. Up." You hiss again, and now she slowly, shakily, stands up. A weak voice under the hood croaks. "General? Is that you?"
