What did change?

He remembered having long hair and wearing jewelry

John still looked the same as before the appointment, but in his mind his own image had changed. He didn't realized that he wasn't looking like he thought he was. To correct that he went from Dr. Valentine's office straight to the mall. Again a lot of people stared at the boy wearing make-up like a girl, but instead of feeling bothered by it, John felt proud that they noticed his beauty. At the mall he went directly to a salon.

"Welcome. Do you have an appointment?" A stylist, a beautiful woman, asked him as he entered.

"No, but I would like to get some things done." John replied.

"Sure." The stylist said a little surprised of the boy wearing such feminine make-up. "What do you want done?"

"I like to get my hair extended to go past my shoulders and get it styled." John answered.

"Are you sure?" The stylist asked puzzled.

"Yeah, and make it a little messy, jet feminine." John said without realizing the stylist found it strange that a boy wanted a feminine hairstyle.

...

John left the salon with the hairstyle he thought he always wore. His hair had gotten extended to reach his chest, was dyed dark brown with lighter highlights, and the sides fringy cut. It was a clearly feminine, jet a little messy hairstyle.

After that John went to a jeweler and bought a lot of fancy earrings and some fancy bracelets. He also got his ear pierced and a pair of fancy earrings put on them.

With his new long hair tickling and the earrings dangling against his neck, John left the mall. He now looked like he thought he did, which meant quite feminine. If it wasn't for his body and clothes, he could have been easily been mistaken as a girl. And it seem his surroundings did it. He was getting less stares as he walked home.
