Shifted in her seat, catching her skirt on something.

Mackenzie focused completely on the test and had made it about halfway through, working faster than most of the other students in the room. But her intense focus betrayed her when she shifted her weight the wrong way in her seat. Unbeknownst to the redhead, the seat she was resting on happened to have a recently developed crack in the wood. And when she adjusted her weight the bottom of her skirt snagged in that small crack.

Throughout the rest of the test, Mackenzie shifted her weight back and forth another half dozen times or so. This allowed her skirt to become more and more ensnared in the grip of the wooden seat each time. And eventually, Mackenzie finished her test and prepared to stand up and hand it in.

Though it felt like an eternity to her, she had actually pushed through the test quite quickly. Indeed, only two other students had handed in their exams and left so far. This came back to bite Mackenzie in the shapely rear, however, as that meant almost the entire class was still present as she rose from her seat. The loud tearing sound that accompanied her movement was deafening in the near silent room. And at nearly the same moment, every set of the eyes in the room were fixed on her aforementioned shapely rear.
